Legal-Guardian Consent for Participants Under 18
Applies only to participants under 18 on the lesson date. 18- and 19-year-olds should not be required to obtain parental consent merely because of age. We do not treat “the minor ticked a box” as guardian consent. The Chinese version is authoritative.
Guardian confirmation (ticked during online booking)
- I confirm I am the person with parental authority or another legally authorised representative of the under-18 participant below, and am entitled to consent for this lesson.
- I have read the Terms, Cancellation & Refund Policy, Risk Disclosure & Participation Consent and Privacy Policy, and understand the inherent risks.
- I consent to the minor taking part and confirm I have truthfully provided important safety-relevant health and behaviour information.
- I understand this does not waive the School’s liability for intent, gross negligence or anything law does not allow to be excluded.
- Where the lesson requires it, the minor should also receive risk and safety explanation appropriate to their age and understanding.

- Minor’s name and date of birth
- Guardian’s name, relationship, phone, email
- Emergency contact (if different)
- Corresponding booking ID
Portrait consent (separate, optional)
- I separately consent to use of the minor’s image per the Portrait Consent. Not ticking this does not affect participation.
